Match summary
Luis Suarez headed home after just three minutes before an unfortunate own-goal from Layvin Kurzawa five minutes before the break.
A Lionel Messi penalty in the 50th minute gave Barcelona real hope of overcoming their 4-0 first-leg deficit until Edison Cavani pulled a goal back on the hour mark.
Neymar scored a superb free-kick with two minutes to play to reignite the dying minutes, and was then successful from the penalty mark just three minutes later after Suarez went down in the box.
But it was substitute Sergi Roberto who completed the unbelievable turn-around when he got on the end of a Neymar delivery to smash home for what proved to be the last kick of the game.
Full report
With a mammoth task ahead of them, Barcelona started their comeback inside three minutes as Suarez headed the ball over the line with his back to goal in front of PSG goalkeeper Kevin Trapp after a good ball into the box from the right.
It was an unsettled, disjointed start from the visitors, with their hosts dominating in dangerous areas.
Javier Mascherano fired just wide on the volley either side of a couple of handball appeals in the box of both sides that were waved away by the referee.
Samuel Umtiti made a last ditch tackle to stop Cavani and Lucas Moura from racing through on goal ahead of a Messi free-kick that flew just over the target midway through the half.
Neymar tried his luck with a long-range effort from the left on the edge of the 18-yard area, and had Trapp scrambling as the ball fizzed inches outside of the right post.
The home side were fully in control but Messi couldn't add to the scoreline with a free-kick he smashed into a PSG defensive wall while Andres Iniesta's strike from range was always rising.
Moura should have made more of a lapse in concentration from the Barca defence with a shot that failed to trouble Marc-Andre ter Stegen as both Rafinha and Suarez may have done better with similar efforts from wide on the right.
But Barca we two-nil up on the night on 40 minutes when Kurzawa sliced the ball into his own net after a backheel from Iniesta on the byline as PSG failed to deal with a punt forward, allowing Suarez to lift the ball into the box over the top of the PSG defence for the Spanish playmaker to chase.
And it got even worse for the visitors at the start of the second half when Messi buried a 50th minute penalty after Thomas Meunier impeded Suarez as he dived in front of the Uruguayan in the box.
Cavani so nearly scored a vital away goal for his side on 52 minutes when he connected to a Meunier cross after getting the better of Neymar, but the effort rattled back off the right upright.
But the lethal PSG frontman had his goal with 62 minutes on the clock with a superb curling shot with the outside of his right boot after Kurzawa had done really well to knock down a long ball forward under pressure from Ivan Rakatic.
